Everyone’s journey to recovery is different.  

Some clients have just received a diagnosis. Some have left hospital and are working towards independence. Others are stuck in a community placement with little prospect of moving on.

We understand the challenges they face and create a support package to meet their needs, whatever stage they are at with their recovery. 

The pathway we recommend is designed to be flexible. The pathways grow with our clients as their requirements change.  

The quality of care delivered has been excellent, the economic argument is a powerful one comparing the cost of placement in secure hospitals with the scheme provided at the TILT project.

Our support services draw on:

  • The skills and experience of the Bridge team
  • Robust risk management and planning
  • All of which are built on a foundation of continuous development through shared learning and practice with the Bridge team, with people who have lived experiences of mental health problems, and our partnerships with leaders in the field of mental health.

We currently operate services in South East London, North East Essex and West Essex.

However, we are not limited to these area and are very happy to discuss developing the service elsewhere in the UK.

What makes us different?  

  • Unlike other high support services, our service focuses on giving clients the skills they need to live independently 
  • Our aim is for clients to move on after 6 months, to medium support or independent living with a low level of support 

The service will also benefit from an in-house ST&R Worker who will focus on supporting service users in their recovery journey with an emphasis on providing time and support to access community resources, employment, education and training as well as helping to maintain their physical health. There main responsibilities include:

  • Working alongside the OT to ensure that a wide range of structured activities are available to the service users on a weekly basis. Taking the lead on supporting service users to access community resources to develop resilience, help them live fuller lives and move on towards greater independence
  • Engaging service users in therapeutic activities, monitoring mental wellbeing and progress
  • Engaging in physical health and wellbeing initiatives which will support service users with their individual health needs
  • Focusing on the emotional, psychological, social and spiritual needs of service users

Bridge currently operate the Integrated Community Enhanced Rehab service within the borough of Kingston. However, we are not limited to this area and are very happy to discuss developing the service elsewhere in the UK.
