Our Flexible Support Service bridges the gap between medium support and independent living. The right support that is little but often, and crucially, on demand, can be all that is required to prevent a mental health crisis for individuals at such a critical stage in their recovery.
Medium support – the stepping stone between 24-hour support and independent living
If the steps immediately taken upon leaving a 24-hour mental health service are too big or too fast, there is a real risk of clients experiencing a relapse and being readmitted to a more restrictive style of care. Our Medium support, which is a gradual pathway to recovery, with continued support of a less intensive and intrusive nature, can be the perfect intermediary stepping stone on the path back to independent living.
How our Hospital to Home service could get you home, improve patient experience and reduce the demand for inpatient resources
For people in an acute mental health crisis who would otherwise require hospital admission, there is an alternative. The Bridge Back Home team of experienced workers based with the Greenwich HTT at Oxleas House in Queen Elizabeth Hospital, work alongside colleagues to identify people who could be supported at home, with a particular focus on non-clinical interventions and tasks.
Providing a vital bridge between secure hospital mental health units and independent living
Many people suffering severe mental health conditions are resident in expensive hospital mental health units for far longer than necessary. The nature and necessarily short-term focus of hospital treatment can be detrimental in the longer term. However, we can demonstrate how specialised 24-hour support in a community setting can break this cycle and is far more effective for both treatment and recovery than the support available as a hospital inpatient.
Community Forensic Support Services – cost-effective & proven outcomes
Many ex-offenders with mental health problems who have committed serious offences, find themselves without anywhere to go when they are released from prison. As a result, many end up back in low, medium, or high-security hospitals or alternatively re-offend again. Results from our Forensic Support Services show ex-offenders with mental health problems can thrive in a community setting, and with the right frameworks in place, community mental health services can be safe, successful, and cost-effective.
